Home-Based Educator/Caseworker for families with children under age 5
QUALIFICATIONS: Associate's degree in Human Services, Child Development, Early Childhood Education, or a related field, plus a minimum of two (2) years experience working with children 3-5 years of age and/or pregnant women. Must have a valid NYS driver license and have ability to travel during working hours and occasional weekends (agency van provided).
RESPONSIBILITIES:
- Maintain a caseload of up to 12 families. Discuss with parents/guardians the program requirements and services that are provided to families.
- Conduct weekly 90-minute home visits with assigned families, and collaborate with families in developing weekly home visit plans.
- Complete developmental, social and emotional screenings on all children within 45 calendar days of child's enrollment/entry date.
- Provide individualized educational experiences that are age appropriate utilizing the assessment and observation tools.
- Identify children in need of possible evaluation/service, following the agency's policy and procedure for referrals.
- Assist parents in obtaining well child check ups & medical care for health concerns.
- Incorporate social emotional development into weekly visits with families, and arrange occasional socialization activities.
- Complete required documentation, including home visit plans, socialization plans; assessment plans, and monthly reports.
